At Commonwealth Pediatrics, our goal is simple: keep the focus on your child’s care while being clear and transparent about costs.
Insurance can be complex, and we are here to support you—but we also want to ensure you understand our billing and insurance policies and your role in the process.

Understanding Your Billing and Insurance Responsibility

Most insurance plans include some level of cost-sharing. Here’s a simple breakdown:

  • Copayment (Co-pay):
A set amount due at the time of your visit.

  • Coinsurance:
A percentage of the cost you are responsible for after your deductible is met.

  • Deductible:
The amount you must pay out-of-pocket before your insurance begins covering services.

Because every plan is different, we encourage families to contact their insurance carrier directly with questions about coverage, deductibles, co-pays, coinsurance, or wellness benefits.

Insurance & Coverage

We file claims to your insurance as a courtesy.

However:

  • Your insurance plan determines coverage—not the practice

  • Please verify Commonwealth Pediatrics is in-network with your insurance plan.
  • Any denied or non-covered services are your responsibility

  • Wellness, sick visits, labs, and procedures may be covered differently depending on your plan

Insurance Cards & Updates

To help us serve you well:

  • Please bring your current insurance card to every visit

  • All updates must be shared at check-in

  • Incorrect or missing information may result in charges being billed to you

Accurate insurance also ensures labs, referrals, and prescriptions are processed correctly.

New & Expecting Parents

Congratulations on your growing family! Most insurance plans require that you add your newborn within 30 days of birth.
We recommend contacting your employer or insurance provider as soon as possible to avoid delays or denials.

Co-Pays & Payments

  • Co-pays are due at check-in
  • Self-pay visits are due at the time of service
  • After insurance processes your claim, you may receive a bill for:

    • Deductibles
    • Coinsurance
    • Non-covered services

Accepted payment methods are cash, check, Visa®, MasterCard®, American Express®, and Discover cards. A $35 fee applies to returned checks.

Self-Pay

If you do not have active insurance:

  • Payment is due in full at the time of visit
  • We are happy to provide self-pay rate guidance when possible

If insurance is not provided or verified within 30–45 days from the date of service, the visit will be processed at self-pay rates.

Laboratory Services

Many labs are completed in our office. Some lab services may need to be sent to an outside laboratory, such as LabCorp or Quest, depending on your insurance requirements. Outside laboratories bill insurance separately. You may receive a separate bill from the lab based on your insurance coverage.

Divorced or Separated Parents

The party responsible on the account is financially responsible for charges.

Co-pays and balances are expected at the time of service, regardless of which parent or guardian brings the child to the visit. Commonwealth Pediatrics is unable to manage or enforce custody agreements, divorce decrees, or financial arrangements between parents/guardians.

Billing & Statements

If a balance remains after insurance processes your claim, you will receive a statement.

We encourage you to compare your statement with your insurance Explanation of Benefits (EOB). For questions about how your insurance processed a claim, please contact your insurance carrier directly.

Payment Support

We understand that healthcare costs can be unexpected. Payment arrangements may be available for qualifying balances.

Collections

Balances not addressed may be sent to collections after 90 days.
